Michigan Green School Application Forms
This is where you can download the application form to apply for Green School status. If you are just starting, take a look at the form to get an idea of what your school will be required to do.
On the application form, you'll need to document at least 10 Green School Points. Emerald Awards require 15 points and the highest award, Evergreen is 20 points. Please note:
We offer the form in two formats, as a Word document and as an Adobe Acrobat PDF file. It is recommended that MAC users download and use the Word version of the application.
The PDF file allows you to type in your text, or you can print it out and fill it out manually.The form fill-able PDF will provides check boxes to assist applicant schools in fulfilling the requirements to become a Michigan Green School. You may either print out the completed form and mail/fax it or email the electronic file to your County Coordinator (please check with them to see which they prefer). PDF files require that the freeware Adobe Acrobat Reader or the full version of Adobe Acrobat is installed on your computer (click on the icon below to go to the Adobe site). Please make sure that your Adobe Acrobat Reader is up to date before filling out the Michigan Green School PDF Application. Your organization may require you to obtain clearance before installing software; please check with your IT or computer tech personnel for assistance.

- Application deadline is March 1, 2013.
- The forms must be turned in to your Michigan Green School County Coordinator no later than this date to be eligible.
- Contact your County Coordinator to verify where the form should be sent, and whether they prefer postal mail, fax, or email.

IMPORTANT NOTES: The form fill-able PDF is not recommended for MAC users. MAC users have experienced difficulty viewing and saving their work in the form fill-able PDF version. MAC users should use the Word version of the Michigan Green School application. If you wish to include pictures with your application please use the Word version of the application or attach them to an email to your County Coordinator.
